Every application for enlargement of time for leave or extension of time to appeal shall be supported by an affidavit setting forth good and substantial reasons for the failure to appeal or to apply for leave to appeal within the time allowed. The following documents shall be attached to the application: (a) A copy of the judgment from which it is intended to appeal, (b) A copy of the proposed notice and grounds of appeal, which prima facie show good cause why the appeal should be heard, (c) Where leave has been refused by the lower Court, a copy of the order refusing leave.
